For those who have Shengen VISA with multiple entries: you need also Czech VISA which can be obtained at the Czech Embassies or Consulates.
You will need only passport, one photo and your insurance contract (or kind of proof that you are insured for traveling abroad). Issuing of the VISA will take not more than 5 working days. VISA can cost up to 30 EUR (for some countries it is also for free).
